Your initial consultation with Dr. Maddali will last approximately one hour, and will include taking photos. However, you should allow another 30 minutes for parking, filling out minor paperwork and enjoying a cup of cappuccino in the waiting room.
When you arrive at the office, you will be greeted by our receptionist, who will assist you in filling out a few forms and consents. A brief patient questionnaire can be downloaded here and filled out in advance to save time, if you prefer. Our office strives to be as efficient as possible, incorporating an electronic medical record and scheduling system.
Dr. Maddali performs all non-surgical procedures including Fraxel, Silk Peel, Botox and injectables in his office, as well as laceration repairs, scar revisions, and other minor procedures. For the comfort of our patients, we have a state of the art exam and procedure chair. However, for the safety of our patients, we will not do any procedures requiring sedation in the office. Dr. Maddali has operating privileges at several local hospitals, including California Pacific Medical Center, St. Mary's Medical Center, St. Francis Memorial Hospital, Seton and Mills Peninsula Hospitals.
We have partnered with The Post Op Recovery Place, which provides personal, private, 24 hour, non-medical care for our post-operative patients who would like to recover somewhere other than home or hospital. Although street parking can be limited, there is ample metered parking on Sacramento and California Streets, and free 2 hour parking on Laurel Street and other parallel side streets. Metered parking here usually has a 2 hour limit and that is sufficient for most appointments, except for Fraxel treatment. Additionally, there are several affordable garages in the area:
Jewish Community Center - 3200 California Street (located on the corner of California and Walnut). 2 hours = $6.
Garage - 3490 California Street (but actually on Locust Street between California and Sacramento streets). 2 hours = $9. This garage has no name and fills up quickly.
University of California at San Francisco (UCSF) Laurel Heights Campus - 3333 California Street (located on the corner of California and Walnut). Public metered lot. 2 hours = $5.50.
California Pacific Medical Center (CPMC) - 3698 California Street (located on the corner of California and Parker). There are three other CPMC garages which are also close by, listed in order of proximity to our office: 3905 Sacramento Street, 460 Cherry Street, and 3838 California Street).
All patients will be charged a $100 consultation fee, which will be collected at the time of your initial consultation. Should you choose to have your surgical or non-surgical procedure performed by Dr. Maddali within the next 3 months, this fee will be credited to your account..
We know that things come up, and simply ask that if you need to cancel a surgical procedure (NOT injectables, Fraxel, SilkPeel), you please do so at least 5 business days (1 week) in advance. This allows us to open that time slot for our other patients and free up the Operating Room staff and schedule. Consequently, if you cancel in less than 5 business days (1 week) of the procedure date, we will charge a cancellation fee of $500. This fee will be credited back toward a procedure or treatment that is booked within 3 months of canceling the prior surgical procedure.
Dr. Maddali is an out-of-network provider for most insurance companies. We will submit claims for our reconstructive surgery patients and happily assist in getting reimbursements from their insurance companies.
For cosmetic surgery, payment in full is expected 5 business days (1 week) before surgery or in the case of an in-office procedure, at the time of the procedure. You can make payments by check or cash. Additionally, we accept Visa, MasterCard, American Express and Discover. Payment by check that is mailed to the office in amounts over $1000 needs to be by a Cashier’s check.
We understand that payment in-full is difficult for some of our patients. In order to provide as much help as possible, we offer financing options from MyMedicalLoan, CareCredit and Capital One.
